I have gone through the Prospectus 2022 & Rules & Regulations for the course written in back side of this Application Form and shall obey the rules and regulations of the course now in force and as amended or altered from time to time. I shall abide by all rules and decisions of the authorities in all matters of course/ training.

Declaration by Parent / Guardian (Parent / Guardian’s declaration is not mandatory for candidates have age 18 and above) I have gone through the Prospectus 2022 & Rules & Regulations for the course. Hereby declare that the application is made with my consent. I undertake the entire responsibility of the good conduct of my ward, payment of fee without default, making my ward complete the prescribed course.
